Requirements for PRE-SCHOOL
1. PSA Birth Certificate (Photocopy)
Kindergarten - 5 years old (until October 31)
Requirements for Incoming Grade 1
1. Original Form 138-A or Original Report Card (Kindergarten)
2. Form 137-A (Kindergarten permanent Record)
Grade School Transferees (Grades 2 to 5 only)
1. Original Form 138 or Report Card
2. PSA Birth certificate (Photocopy)
3. Certificate of Good Moral Character
Requirements for Incoming Grade 7
1. Grade 6 Report Card (Original and 2 Photocopies)
2. PSA / NSO-authenticated Birth Certificate (Photocopy only)
3. Certificate of Good Moral Character
4. Four (4) pieces of 2x2 picture in white background
5. Brgy. Clearance of Parents
Junior High School Transferees (Grades 8 and 9 only)
1. Original Form 138 or Report Card
2. PSA Birth certificate (Photocopy)

4. ESC Certificate (ESC Grantees from other Private schools)
5. Two (2) pieces of 2x2 pictures in white background
